To state the obvious, Lew Alcindor --> Kareem Abdul-Jabbar. I think a few other NBA players have adopted Islamic names, right? There was a guy in the early '90s if I recall.

He's Ed Mathews on his Topps issues but the 53 Bowman I have lists him as Eddie Mathews.

Chris Jackson/Mahmoud Abdul-Rauf. Another Basketball is Keith Wilkes/Jamaal Wilkes. A couple of Football, Bobby Moore/Ahmad Rashad, Sharmon Shah/Karim Abdul-Jabbar. Kareem Abdul-Jabbar later sued him and he changed it to Abdul-Karim al-Jabbar.
