Just saw posted online that a strip card I submitted to SGC that had a PSA 4 and was to be slabbed "same or greater" has been labeled "O-G." I can see from their guidelines that apparently this did not meet the minimum, which was a 4. Does that mean 1) they simply left it in PSA holder 2) slabbed in SGC holder at lower grade, though advised otherwise? 3) left raw? Thanks.

It just means that the minimum grade you requested was not met. Yes, they would have left it in the PSA holder. Once you get the card back in hand, the grader will probably have marked on the card (usually on a graded card sleeve) what he saw that kept it from crossing over.

The submitter makes the decision to allow them to crack the slab or not. If you don't want to take the risk, they can't be expected to open the slab without your permission.

If youve submitted for crossover, I would have guessed that you are implicitly saying that you are ok having the slab cracked...as they cant regrade your card in the slab.

Me too. I have found some serious surface blemishes that weren't visible thru the case and can cover light wrinkles as well.

what if they think it looks like a "4" (or whatever minimum grade it was requested at), open it up and realize there is a crease (or paperloss etc) they couldn't see thru the plastic and its not really a 4?

I don't know for sure if the answers you got above or right, but I'm pretty sure they could tell just by looking at some cards through the holder, whether they think they are overgraded or not. We've seen plenty of cards posted here that are graded poorly. I have a PSA5 with paper loss on front(Old Judge card). If sure if I said don't grade it if it won't get a 5, they could tell right away it's not getting a 5 without taking it out.
Possibly, since you said it is a strip card, they may think it isn't big enough to grade. Since strip cards were hand cut, it's all very subjective anyway
