How can this card not get a MK or ST qualifier? Is OC the only qualifier you can request to have have listed as part of the grade?

Staining can be pretty subjective. I've rarely seen PSA use it except for wax stains. For example, just about every 1914 Cracker Jack would have the ST qualifier if they were strict on using it. In general, I believe, if PSA feels the mark came from a pen (or ink) or pencil or marker, then they will put the MK qualifier. However, if they believe the card is just dirty, has fingerprints, etc, then they will not. It's hard to see the picture but they may have thought that card was just dirty. If that happens to be an ink stain on back however, then they just missed it.
