Back scan will be posted when I get a chance - nothing to speak of looks like the front, all lettering clearly legible.

@Ladder7 - Don't know what you ate for breakfast today but I can do without the "Douchebag" comments via this and PMs - I don't even know you, so please save that for someone you have actual beef with. Yikes, you honestly scare me.
