Regarding the question on scanning, I'd bet my bottom dollar that, based on your description, you have one of those super thin Canon scanners that uses a CIS scanning element. The nature of this type of scanner is that it must be in very close contact with the item being scanned in order for a clear, crisp image to be created (and thus a grading holder or any other thing that causes the card to be raised off the glass is a problem). The other kind of scanner, common in HP brand scanners, which are typically much thicker and larger, is a CCD sensor. The CCD sensor is able to accurately focus on things that are further from the scanning glass because the sensor is further away from the item, and it works more like a camera.

I went quite a while with very blurry scans on graded cards, and any card with any sort of plastic protective older, before I realized the problem. Unfortunately, the only solution is to get a different type of scanner. Luckily, I've seen the HP CCD-type scanners frequently at Staples for about $50, and sometimes even cheaper by rebate or what have you.
