I've torn the tab off a Hires and a T201 so I've been wanting a solution to this. When using the ticket holder, is the card penny sleeved first so that there is something to grab to pull the card out of the holder?

I'm not aware of a penny sleeve for ticket size toploaders,. But I don't pull cards out of toploaders, I turn them on their side a tap the side until the cards starts to come out. Then it just slides out easily.

BCW does sell large currency penny sleeves that measure 7 7/16 x 3 1/4 which fit tabbed hires
https://www.bcwsupplies.com/currency-sleeves-large-bill Both ultra-pro and bcw sell the large bill top loaders for these as well. Both top and side load.
