You can look back in past posts, I saw two in the past-
1) An Alexander Cartwright autograph that sure looked like it was an Onion Skin copy. 2) A Charles Comiskey autograph that was actually and Charles Comiskey II autograph I don't think think either of these were actually slabbed, but the cards were incorrect.

This happened years ago with a different company, but the worst I ever saw was an Eddie Plank card with a ballpoint signature of the 1970's Giants pitcher of the same name! I don't recall which company this was, but could they please hire someone with half a clue?
