Unfortunately my memory isn't the best anymore so can't remember the exact event it was handed out at. Hopefully Jon will give the answer.
Here is mine that I was fortunate enough to have a fellow member get signed for me at a event with Mr Ripken. The one with my name is authentic and the one with my name removed is a custom I made.

The #B1 Dipkin was issued in The Simpsons comic #4 in 1993/94. The card was inside the comic within it's own plastic wrap.
The #P4 Diamond was a promo given out at Comic Con or something similar around 1994 as part of a set. A promo set. Great parody cards for the FF collectors!
