Same thing has happened to me with 1974 Topps stamps. The last one I got, just a few months ago, the stamp itself was turned 90 degrees, and was completely out of, and above the part of the slab it was supposed to be in.
It seems that the thin paper items that do not completely fill the slab, are particularly sensitive to shaking and bumping by the mail system. PSA really needs to rework the slabs to better hold the thinner items. Steve
