SGC used to note printing errors on the flip. But, I heard as of the last year or two, they weren't anymore (rumor, truth, not sure. Maybe others can join the conversation)...
I was the one who convinced SGC to start adding printing anomaly notes on the labels. I had them note the very first one (I believe it was Elberfled, that was missing ink), circa 2005. PSA for a while followed suit and was labeling errors, but that might've stopped as well. Re: adding a collector's name, I don't think they do that often, rarely. Would need to be a very important collection or big find.
