Has anyone ever seen this before ? Koufax Mantle Boyer 1965 transfers

I have no luck posting pictures so I need someone to please post the pix for me ( so I guess no one has seen it ! )

When you do a google search and check the Robert Edwards Archieves and nothing comes up as a match I figure it�s pretty rare or new to the hobby.

I have been collecting since 1971 and have never seen it. I picked it up at the National show in Chicago. I bought a � empty � 1965 topps baseball 5 cent box and it was thrown into the deal as a companion piece.
Any thoughts or info would be great.
