Default BGS or PSA ever "lose" anyone's card(s)?

Hi everyone. New member here -- excited to join. Thanks for reading my post, and sorry in advance if this has been a topic of discussion before!

Am curious if anyone has recently or ever sent a card(s) to BGS or PSA or others and then been informed that they've "lost" or "misplaced" the order. By that I mean the company itself lost the order -- not in transit. Can you share any details about your experience? Or of similar stories you've heard but know personally to be true?

Asking because of some press interest in the frequency of this and the potential for fraud, so trying to get a sense anecdotally from members just how common it is.

Never happened to me in the past.

I did have one time when PSA pinched one of my cards during the slabbing process, and gave me a credit for the value that I ascribed to the card when I submitted it. But they've never lost anything.

PSA internally misplaced one of my orders for 2 cards. I do not recall the service level. They eventually found the order, graded the 2 cards and returned them. This occurred during one of their service level "specials" so I am sure the order was lost in the shuffle and they eventually figured out what happened. The overall delay was 6 months on top of their normal turnaround time for the service level.
