Not really a "template" but I use a SGC gasket from a cracked slab to line up even scans. For graded cards I'll place the gasket against the bottom edge of the scanner and just slide the slab up against the gasket. Same for raw cards, I'll almost universally scan them in a One Touch pressed up against the gasket.
For add size cards I don't have a recessed holder to scan them in I'll placed the gasket against the right of left side of the scanner about half way and level the card against the gasket. Then I'll use a business card to lift the gasket off the scanner bed and 99% of the time you have a level scan.
