7) Was the attached card provided along with 5-card packs of Glossy Send-aways?
The prevailing thought I have seen is that they were returned with the giveaways. I simply do not remember personally to verify. There were also three variations of the checklist, Uncut sheet, Collector Boxes, and penny sleeves. I don't have them but photos are on TCDB - https://www.tcdb.com/Checklist.cfm/sid/10117
