Hard to tell. I see your concerns but wouldn't make a guarantee from what I can see on the scan

I think it was likely tampered with based on the cloudy edge, wavy PSA tab, and it also looks off center in the holder to me.

My opinion, card/holder are fine. Way too much talk here about frosted edges=tampered. Not the case in many instances. Just got a sub back from PSA, several had "frosted edges" and a few with a wavy label.
