An eBay seller has a number of these stamps available, all removed from an album along the way, and you can even see its "5" mounting spot lingering on the back. (I picked this up for my type collection.)
Jerry Lumpe played for NYY 1956-1959, so that basic date makes sense. Anyone have a more specific year or know its set designation? Actual size is small, about 1.25" x 1.5".
