Good old "Huggie"

I used to collect Hughie Jennings cards. Now, thanks to PSA, I am going after the cards of his long-lost brother. Anyone else? How on earth does this happen anyhow? I understand someone could hit the wrong button and maybe the card gets labeled "one hand showing" instead of both, but seriously, are people really sitting there re-typing the player's names when cards get graded?
