1940s Andy Lotshaw Body Rub Premium
One of the fun things about collecting type cards is the places it can veer to. Andy never made the Bigs but did play for 13 seasons in the minors, according to Baseball Reference. Here is a premium for his Body Rub I classify under the ubiquitous ACC # for advertising, "H"-

Very cool premium! Several years ago, I purchased an Andy Lotshaw Cubs photo pack. Other than that item, I had never seen his name in connection with baseball "card" type items. The background on your premium even looks a little like the photos in the pack. I wonder if he distributed both himself?
