Bought this couple years ago
Label was loose and holder opened on side. Is this the correct card ?

Lol - yes..thats the correct card. Grade seems kinda generous considering how extremely round those corners are, but seeing as how the lowest they can go is a '1', well...........the loose side would have me worried,
