This lineup card style is pretty uncommon to see. From what I have seen over the years, I don't think many teams bothered with a lineup card for the official scorer, although the Padres were not the only one (the Orioles actually had a lineup card carbon pad with 4 sheets vs what most teams had with 3 for a period of time) with the back copy for the official scorer.

IMO Gaylord Perry is one of the most underrated pitchers in baseball history. Here is the game where he passed Bob Feller to move into 2nd all-time in K's. He would subsequently be by quite a few including Nolan Ryan, Steve Carlton, Randy Johnson etc but pitching in an era where K's were far less common, Perry was dominant.
