While I wasn't the one who pulled it, the card pictured with the Obak pack above came directly out of it. I've also seen someone pull a triplefolder from a Hassan pack, and know that in the 1980s, a few board members opened some Honest Long Cut packs for the T205s.
In addition, there is the famous Old Judge story where a man pulled an Old Judge card directly out of the pack at the East Coast National (I believe). SGC graded it on the spot as a 98 Gem Mint 10.
