The simple solution is to put a small piece of paper of the opening of the toploader and then tape it on there. The card cant come in contact with the tape and it won't slip out of the holder.

My biggest pet peeve is when someone uses a non rigid type holder or just a pvc sleeve and then tapes that onto a piece of cardboard.
