It does indeed mean that it is the first card they graded IN EACH GRADE. So it's possible to have a 1938 Goudey Bobby Doerr GAI 1 First Graded, and a 1938 Goudey Bobby Doerr GAI 2 First Graded.

It means absolutely nothing. It is, to quote another thread, puffery.
