This is a spin off from the thread - "How did this get a PSA 7?"

Can we get a list of the worst screw ups by the major grading companies?

Note that I like picking up PSA screw ups. I think the coolest one so far is a T201 that is folded and encapsulated in one of the modern day card holders. The biggest error I have is an N172 of Sunday with the ad bottom cut off.

The best errors by the grading companies (only PSA, SGC, SCD, BVG considered). Jury is still out on GAI (at least for me). I'm pretty sure we can leave out AAA, PRO, ASA and a few others.
