PSA does this sometimes and they call it a Mechanical Error. Or they just type in the number wrong and call it a cert number, like the 2x Joe Montana rookies they labeled PSA 10 that were both at least 70/30 off-centered. The submitter who got them posted about it, and PSA recalled those cards and gave them 6s the second time.
The bigger scam that happened last year with CGC comics grading side was that some submitters were taking advantage of their reholders and getting previously lower graded comics slabbed as higher graded comics because they submitted them as accidentally cracked out from the outer slab, but then they went and re-submitted the original high-grade comic raw to get a 2nd high grade copy. Then they kept doing that.
