I believe that and a similar Mantle were premiums from a 1960s Alvern Batting Game. The third photo is the basis for saying they're from Alvern. It's a photo a friend picked up from Kelly Eisenhauer. Although, I'm not sure who put the Alvern notation on the photo. I also have the same Mantle photo with slightly different cropping without the facsimile autograph as a Type I Requena photo, so I suppose they are both based on Requena photos.
