I don't have photos or answers to most of your questions, but I do have the pack odds information on the golds.
There was one gold in every 36 wax packs (one per box), one in 18 cello packs (24 pack box), one in 12 rack packs (24 pack box), one in 6 jumbo packs, 5 in every vending box, and 10 in every factory set.
