This 1979 design doe remind me of the 1968 Topps 3-D test issue (which itself looks like the basis for the 1969 base set).

David, is this Hernandez similar to the 1972 Topps Clemente base card mock-up as it was likely a 1/1 just for presentation purposes? I wonder if another player was designed / printed like this. Guess the only difference is 1972 was a design candidate for the base sets while this 79 was a presentation test?
