Now that I have the item in hand I don't think it was issued with the scorecard. First the crease on the front doesn't seem to affect the scorecard inside. And second the scorecard is pasted inside the Blue cover but there is
a scorecard on Ebay without Blue cover that doesn't have any damage on the back page . Those two things Lead me to believe that the Blue cover was a fan giveaway and is seperate from the scorecard.
