Well, could be a couple different issues:
1) Real card in a counterfeit slab.
2) Counterfeit card in a counterfeit slab.
3) Original card and flip removed from PSA holder and used to put an inferior card in a new counterfeit slab and re-grade the original card.
4) Every once in a while, PSA got two of the same card during an order and gave them the SAME cert number.
At least with eBay Authenticity Guarantee, if you meet the requirements, the card will be sent back to PSA to verify that the slab is authentic and the seals are still intact. If not, PSA will notify eBay to cancel the sale.
