I'm guessing a lot of the proof sheets back then were just mocked up by hand. I know they were in '67, what with the whole "Tom" vs. "Tommy", "Tommie" Reynolds card and the weird spacing that wound up on the final version. Keith Olbermann has a whole article on that somewhere.
