 If it was just the tear I would think it would just say 
AUTHENTIC, yet this has a designation of 
ALTERED added to it. I haven't seen that added to cards that are just too low of a grade to be a 1.  There has to be something else, or a mistake by PSA.  Based off the condition, though, I am sure whatever the "alteration" was, it wasn't done to deceive.
