I'm not sure how they were issued but since they were premiums I would assume they were acquired thru the mail either by a redemption program or cash.
Not to hijack my own thread, but I looked on Google to find the answer maybe 15 minutes after making the original post, and this thread was already on there.
I didn't realize Google was that fast.
