1955 Sandy Koufax Rookie Card #123. It is an amazing card, almost perfectly centered. It should grade very well.
If you look closely, there are two tiny hair-line creases on the front, both coming in appox 1/2" from the sides of the card, exactly at center of each side.
They are very difficult to see and are only visible under certain types of lighting. I took quite a few pictures just trying to get one that would allow you to see them.
It also has a small corner chip in the top left corner.
