PSA, SGC, BVG or SCD worst mistakes...

I think the coolest one so far is a T201 that is folded and encapsulated in one of the modern day card holders. The biggest error I have is an N172 of Sunday with the ad bottom cut off. <BR><BR>The best errors by the grading companies (only PSA, SGC, SCD, BVG considered). Jury is still out on GAI (at least for me). I'm pretty sure we can leave out AAA, PRO, ASA and a few others.<BR><BR><BR>

This is the third ripped front card graded a PSA 3 or 4 I've seen recently, though the only one with the player's picture actually damaged.<BR><BR>I just don't understand how this could happen. Even if the grader was drunk, he would have to notice a rip on the front of the card. And isn't a second grader supposed to look at the card, even if only briefly, for quality control purposes? And if a rip can be overlooked by mistake, how about a massive crease, a clipped corner, or even a counterfeit card?<BR><BR>Is it possible that these overgraded cards are not oversights, but favors?
