Any help on finding the captioned card much appreciated. The search has been long. If I can find one my quest will be over. There is but Uzit known and that belonged to our beloved and late Teddy Z.

7 of them exist between PSA and SGC, apparently. One sold in December in a PWCC auction, a PSA 3.
