Have this coming my way in the next few weeks. Never seen by me before

Anyone have or seen this 1960 Lake to Lake Dairies brochure showing a Stan Lopata card that was not issued as part of the set? Sorry for the poor photo. That is all I received. Will post better scans of both sides once it is in my hands

I understand some resourceful collectors have taken it upon themselves to "create" their own Lopata card based on the one shown here.
