Need help with ID on this Roy Campanella photo/card......

Not sure what year this came from? It has his playing day stats so it could be 1958 but has anyone seen this before? 3.5 inches wide x 5 inches tall. Facsimile Auto.
Any help is appreciated! Thanks in advance!

Looks like a 70s issue to me. It lists his HOF election year of 1969, so no earlier than that.

These were sent to you in the mail if you requested an autograph from him. I received one in the late 1980s (1989 I think)
