Autograph help!!!! Item #1 Help On 3x5 index card auto
 

Have 4 items I need some help on with the autograph experts here, so I will do one at a time until I hopefully have all mysteries solved.

No clue who this is, any help appreciated

One bump for the Monday crowd? Anyone have any thoughts? Most of the autographed items in the collection are from players of the 60's and 70's with a few earlier.
