1955 Sandy Koufax Rookie Card #123. It is an amazing card, almost perfectly centered. It should grade very well.

However, if you look closely, there are two tiny hair-line creases on the front, one on each side of the card, approx 1/2" in length. They are very difficult to see and are only visible under certain types of lighting. I took quite a few pictures just to get one that would allow you to see them.
