There was a long discussion on the PSA boards about these recently. I'm not sure I ever saw any real information in it, but there was a lot of bickering. I think I learned more about the card from your couple of sentences.

I have one. Bob Lemke noted on CU that he felt it was a Topps issue, but as I recall he did not listed it under Topps in The Standard Catalog. I could be wrong about that

Al, It was never listed at all in the Standard Catalog. We never had any information about its origins or distribution.

The OP's recollection sounds very reasonable.
