Default 1983 Topps Send-Away Checklist

Were these sent to the recipient along with the glossy cards? Did one come with any group of 5 you ordered?

If anyone can tell me how one came about these in '83, that would be tremendously helpful as I have never seen one before.

First thought, I had assumed one of these offer inserts came in each pack. Is that not the case?

Nope - not a pack insert unless extremely rare.

They really pushed uncut sheets that year, even getting them into retail.
