Default 1964 fortune shoes Willie mays sketch

Does anyone have any knowledge on the 1964-68? Fortune shoes mail in premiums? I recently acquired a mays as it’s something I had never seen before. My google research brought me to an article written by BoB Lemke discussing this issue. Outside of that, I am clueless.

-I know they were advertised in Ebony/Sporta magazines.
-football/baseball checklist
-9”x12”, blank back
- sketch from famous illustrator Ed Vebell


Any further information?
