The dealer I got it from said it was the 1981 ACC-tion game set but I've not been able to find anything on that.

clearly a dice based game like Strat-o-matic (hence your 2-12 field on the reverse for each event)

I'm guessing one of the Negamco games

EDIT: having trouble finding images of the cards but PTG (Pro Table Games) also had an early 80's dice and player card game.

Hi again, Adam -- Jeff Downey confirms the card is indeed from ACC-Action Basketball, made from 1980 to 1985.
No information yet on manufacturer/publisher or designer/inventor, but Jeff is going to make some inquiries.
