Lost in the Mail: Survey

Reading the most recent thread about a card gone missing (and thankfully found this time) I wanted to conduct a survey about cards that you have lost in the mail. Please answer the following few questions if you have lost a card in the mail (there are certainly many more questions that are relevant but I am trying to keep it simple):

1) Who did you ship with? (USPS, UPS, FedEx...)

2) How did you send the card? (envelope, bubble mailer, box)

3) Did you include a packing slip/business card? (If "yes", was it taped to the card holder?)

4) Did you buy insurance for the package?

5) Was the label completely covered in shipping tape?

6) Did a partial package arrive with no contents or wa there no package at all?

i've never had a card lost in the mail...other objects yes...cards no!

1- 4 cards to Beckett Grading, using USPS
2- in small brown box (cube-like)
3- had a check included inside with submission form
4- yes, insured
5- address written in dark purple marker
6- unknown last whereabouts Atlanta sorting factility
RESULT: file insurance claim and got $$.

Several years ago, I sold a T330-2 Piedmont Art Stamp of Chief Bender to a fellow Board member:

1) USPS.
2) Envelope.
3) No.
4) Yes.
5) No.
6) Completely disappeared while intransit.

2. Cube Cardboard box wrapped with brown tamperproof tape

3. Submission to PSA. (Submission form)

4. Insurance $1500

5. Labeled covered with tape

6. Package never arrived last signed for in St.Louis contained 10 T206's and 5 E254 Colgan Chips. Insurance was paid with no problems.

7. Cards were graded and were lost while being returned to me. PSA gave me a voucher for 15 free grades.

I have had two issues with shipments

The first wasn't cards, it was a group of books that was
1)send by usps
2)box
3)no labels inside, only the outside label
4)no insurance
5) Label was secure
6)The box split open, I received the box completely mummified with usps tape containing about half the books that were sent along with a few items from other peoples packages.

The second issue was a small box of cards which went missing for about 40 days but did end up arriving. No damage, no explanation.

I have had only one card lost in the mail. It was a modern 1-of-1 that cost about $40.

2) How did you send the card? (envelope, bubble mailer, box) Sandwiched between cardboard (usually with bubble wrap) and mailing envelope. Sometimes boxed for small sets or more valuable singles.

5) Was the label completely covered in shipping tape? The address and my return address were covered with tape but not the tracking information

Final comment: We have actually "lost" a half dozen parcels in several years of eBay selling and more than one indicated (via USPS tracking) that the parcel was delivered. We most recently (last month) lost one somewhere between the Puget Sound and Greece. USPS tracking is of dubious value. (We once had a very valuable package - sent registered mail - errantly delivered to a neighbor. Fortunately, our neighbor is honest and delivered it himself to us. Had he been less than honorable, USPS tracking showed it was delivered and the delivery man (one of those much-hated subs) quite possibly would not recall where he delivered it.)

2 priority box
3 Included packing slip, but nothing was taped to the card.
4 No insurance. Buyer had option, but didn't pay for it.
5 It was probably 10-15 years ago. No label, I just wrote the address on the box back then.
6 Buyer contacted me and said the priority box was delivered, but the flaps were wide open and the box was completely empty. I just used the self stick flaps and they popped open. I've taped them all shut since. I'm guessing that PSA 9 Bonds rookie ended up in some postman's collection.
