Default TPG grading mistakes

How often do you run into cards that are slabbed incorrectly by the TPGs? The following is for an active auction on fleabay.

I get it - people are human and make mistakes but if you're going to operate a company that is supposed to be responsible for assigning a subjective opinion, then at least make sure that the card can at least meet a few basic criteria. In this case the card should be in an AUTH holder.

Please post other fun little surprises that have been encapsulated.

It's PSA what do you expect.
I recently cracked open a PSA 7.5 Ted Williams and it was pressed and trimmed. It was pressed so hard that the card is 3 thousandth thinner than it should be. I am at the point I assume all PSA cards have some kind of problem.

The grades that the TPGs give to Old Judges are all but worthless. Not only because of the many trim jobs that they’ve given a number grade to (and conversely the many perfectly fine cards they’ve graded Authentic), but their grades don’t take the image quality into account. Super sharp images with scrapbook residue on the back get a 1, and cards with super faded images get a 5 or 6.

TPGs are horrible at grading OJs. Yup, you can have a well centered card with 4 razor corners and barely see the image and have yourself a "7" grade.

Anybody want to guess what's wrong with the following:

Perhaps PSA just needs to add a QA Check 3 and QA Check 4 step to catch these? Keep in mind, this will add two more months and an additional $100 fee per card.
