Default Lost in the Mail: Survey

Reading the most recent thread about a card gone missing (and thankfully found this time) I wanted to conduct a survey about cards that you have lost in the mail. Please answer the following few questions if you have lost a card in the mail (there are certainly many more questions that are relevant but I am trying to keep it simple):

1) Who did you ship with? (USPS, UPS, FedEx...)

2) How did you send the card? (envelope, bubble mailer, box)

3) Did you include a packing slip/business card? (If "yes", was it taped to the card holder?)

4) Did you buy insurance for the package?

5) Was the label completely covered in shipping tape?

6) Did a partial package arrive with no contents or wa there no package at all?
