According to the pop report, there have been 3 of the error cards graded by PSA and this is the highest graded of the three (the other two are graded 1 or 2). There have been 56 of the corrected versions slabbed.

Yes, the card I have is the only known example like this. Before I moved from CA to MN, Mark Macrae looked over the card with me. He said that it appears that part of the die for the "m" broke creating the "nl". There is no reason other examples couldn't exist, but so far, this is the only one.
