I gave them 2 dated inventory lists (one for business inventory and the other for the personal collection). It included the card issue and player, the grade (since all of my stuff is graded that limited the subjectivity) and the amount I paid for each card. For the personal coverage they require you to schedule seperately any card that cost more than 5k.
