I get cardboard boxes for my graded cards from SGC - I pick them up when I stop by their offices to drop off submissions so that I can save the shipping cost.
I bought my graded card sleeves from an eBay seller - I think the ID was dreamjoey. It was $2 for 100 of them with reasonable shipping, if I remember correctly. It was a few months ago.
